Clinical, anatomical and histological diagnosis

Abstrakt:

Histological diagnosis: Meningoencephalitis metastatica. Autopsy examination of 14-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained by Hematoxylin-eosin, cresyl violet, mucikarmine, PAS, alcian blue and Heidenhain’s azan.In the meninges of the entire CNS, inflammatory granulation tissue composed mostly of histiocytic and plasmocytic cells with few epithelioid cells, lymphocytes, leukocytes, macrophages and Russell bodies. Focally fresh blood was seen. Among both plasma and histiocytic cells multinucleated forms and single mitosis images were observed. A similar type of inflammatory infiltration was present in the neural parenchyma of the cerebral hemispheres in the brainstem, cerebellum, medulla, and in both gray and white structures in the form of granulomas. A significant reaction from the micro and macroglia which is sometimes the primary element of the infiltrate, and formation of large glial lumps, were observed in these areas. Gramm /-/ granules corresponding to bacteria were seen along the vessels and in the neural tissue. In the cortex also necrosis as well as neuronal atrophy and ischemic changes were seen. In Heidenhain's staining in necrotic areas and their vicinity partial or complete demyelination. In addition, throughout the nervous system, significant features edema, congestion, and glial activation and proliferation were observed.
