Clinical, anatomical and histological diagnosis
Histological diagnosis: Encephalomacia multiplices recens et inveterata. Autopsy examination of 78-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin, PAS and van Gieson method.In the corpus callosum there was a fresh focus of malacia covering a very large area. The tissue within the focus was poorly stained, loosened, and single macrophages were seen in some places. Numerous amyloid globules were found in the marginal parts of the fornix. In the basal ganglia numerous foci of malacia were seen, partially destroying the amygdala, globus pallidus, and internal capsule. The foci were in the stage of dissection or glial-mesodermal organization. In addition, perivascular cavities were found. Large atherosclerotic lesions were seen in the vessels. Almost all nerve cells showed features of fatty degeneration.PAS- positive material was present in macrophages, amyloid globules and degenerated nerve cells. Features of edema and congestion were observed throughout the CNS.
