Clinical, anatomical and histological diagnosis
Histological diagnosis: Haemorrhagia cerebri recens et encephalomalacia inveterata hemispherii sinistri. Autopsy examination of 79-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with H-E, PAS and Van Gieson's method.The left frontal lobe showed an old focus of malacia destroying the white matter and partially the cortex. Pronounced nerve cell loss was seen in the vicinity of the focus, as well as ischemic, chronic disease. Intense PAS positive staining was found in the hypertrophied astroglia in the scar. Extensive glial activation and proliferation were seen in the white matter. In the left basal ganglia, a fresh hemorrhagic focus in the dissection phase was present. The vessels throughout the nervous system were vitreously altered, some vessels were only fibrotic. Features of edema and congestion were observed.
