Description:

Clinical, anatomical and histological diagnosis

Abstract:

Histological Diagnosis: Encephalomacia hemisphaerii dextri in regione lobus temporalis et nucleorum basalium. Autopsy examination of 75-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with hematoxylin-eosin, PAS and Van Gieson's method. An extensive focus of encephalomalacia was present in the right hemisphere, partially destroying the lateral thalamic nucleus, internal capsule, white matter of the temporal lobe, and cortex of the hippocampus and lateral occipitotemporal valve. The focus was in a stage of advanced dissection. Dissolving tissue and numerous macrophages were visible in the center. There was white matter swelling around the foci, stimulation of astroglia, and all vessels filled with blood. A symptomatic reaction was seen around some vessels. The cortex around the foci had multiple neuronal atrophy, ischemic, chronic type disease and cellular steatosis. There were features of congestion throughout the CNS and in the meninges. The Virchov-Robin spaces were dilated, the tissue-vascular borderline relaxed. In the meninges macrophages and histiocyte-like cells were observed around the vessels. Pseudocalcium deposits were seen in the choroid plexus
