Abstract:

Histological diagnosis: Necrosis recens bulbi in reg. centralis precipue lateris sin. Arteriosclerosis gr. magno. Autopsy examination of 63-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained by H-E.In the medial vascularized area of the medulla oblongata a focus of fresh necrosis bilaterally at the raphe nucleus, on the left extending in a band across the entire thickness of the medulla, from the lining to the pyramid destroying the area of the nucleus hypoglossi, lemniscus medi-alis and tractus pyramid. sinistri. On the right side necrotic changes were observed lower at the level of lemniscue medialis and occupied only the medial part of tr. pyramidalis. In this necrotic area, decreased pigmentation and loosening of tissue, amorphous disintegration of white matter and cellular atrophy were seen. , Only capillary endothelium and rarely oli-godendroglial nuclei were preserved. No leukocyte reaction or macrophage demolition was detected. In the median fissure and in the raphe nuclei, vessels with thickened walls with symptomatic perivascular lymphocytic reaction were present. Throughout the medulla edema was visible. Severe ischemic damage to nerve cells was noticed, some were also damaged by lipofuscin accumulation. Nucleus n.XII on the right destroyed by edematous necrosis. Basilar artery revealed grade IV atherosclerotic lesions - completely obliterated structure of the vessel wall, vitrification in the middle layer with disintegration of vitreous masses, fibrosis with lymphocytic infiltration. In the right basal ganglia biopsy, there were features of large white and grey matter edema, vessel walls were vitreous and fibrous, perivascular spaces were loos-ened, small perivascular lacunae with lymphocytic reaction in the borderline of striatum. A small degree of cellular atrophy was visible in the grey matter..
