Clinical, anatomical and histological diagnosis
A.Histological diagnosis: Encephalomalacia rubra in regione thalami sinistri. Arteriosclerosis cerebri. Oedema et hyperaemia.Autopsy examination of 84-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with H-E and Van Gieson's stain. In the left thalamus, an extensive encephalomalacia focus with secondary hemorrhage, already organizing were observed. In the center of the focus, structureless masses, hemolyzed blood and vessels with atheromatic lesions and thrombotic changes were visible. In the periphery, proliferation of young vessels were seen. Also collagen fibers, numerous macrophages, many of them containing hemosiderin deposits and gemistiocytic reaction of the glia were present. Blood vessels throughout the CNS were maximally filled with blood. Vascular walls were characterized by vitreous and fibrous changes. Tissue-vascular border loosened, some macrophages and lymphocytic cells were seen. Preserved thalamic cells revealed fatty degeneration. Under the lining numerous amyloid bodies were present
