Clinical, anatomical and histological diagnosis
Histological diagnosis: Encephalomacia partin rubra. Arteriosclerosis. Autopsy examination of 73-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin, van Gieson and PAS method. The vascularized area of the middle cerebral artery revealed fresh necrosis focus, most clearly marked in the subcortical and cortical regions. Extensive cellular desolations were seen in the cortex and partially in the basal ganglia. Preserved neurocytes were altered, predominantly ischemically. Foci of complete necrosis were observed in the white matter, in addition, astroglial activation was evident. Hemorrhages were seen in some areas, abundant especially in the cortex. Features of edema and congestion were noted. The meninges were fibrotic, with fresh blood and macrophages in the subdural space. The vessels showed increased vitreous and vitreous-fibrous changes.
