Histological diagnosis: Encephalomalacia inveterata et recens. Autopsy examination of 63-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained by H-E and Van Gieson's stain. In the right frontal lobe a focus of malacia, destroying the corpus callosum, white matter F1 (with partial cortical involvement) and F2. In addition, there were numerous small foci of degeneration in the cortex of F2. These foci were in the stage of disassembly or advanced glial-mesodermal organisation. At the border of the foci, features of oedema were seen. In the cortex the nerve cells revealed features of fatty degeneration and diseases of chronic and ischemic type were observed. In the meninges a slight symptomatic inflammatory reaction, with single pseudocalcium deposits was present
