Abstract:

Histological diagnosis: Atrophia hemisphaeri sinistri. Autopsy examination of 34-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin, cresyl-violet, Kanzler-Arendt, Heidenhain and van Gieson method.On macroscopic examination, the left cerebral hemisphere was found to be smaller than the right, and the right cerebellar hemisphere was found to be smaller than the left. On cross sections within the left atrophic cerebral hemisphere, it was observed that the ventricle was dilated, the gyri narrow, and the grooves wide. The structure of the cortex was obliterated.The white matter was narrow, and the corpus callosum on the left side was a thin band demarcating the markedly dilated lateral ventricle from above. The staining for the fibrous glia showed its significant thickening. There was a focus of necrotic and calcified glial cells in the white matter of the parietal lobe. The structure of the basal ganglia was normal. A unilateral pyramidal atrophy was evident on cross sections of the medulla. There was a focus of necrotic and calcified glial cells in the white matter of the parietal lobe. The drawing of the basal ganglia was normal. A unilateral pyramidal atrophy was evident on cross sections of the pons.In all examined areas, the atrophied cerebral hemisphere showed extensive fresh and old necrotic lesions in the cortex, with complete or partial neuronal atrophy. Multiple perivascular necrosis were found bilaterally in the basal ganglia. Cellular atrophy was accompanied by cellular and fibrous gliosis. The vessel walls showed fibrous changes, and the meninges were only slightly fibrotic
