Abstract:

Histological diagnosis: Encephalomaciae inveteratae multiplices. Arteriosclerosis crebri. Autopsy examination of 63-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with H-E, PAS and Van Gieson's method.An old focus destroying the basal ganglia, external capsule, forebrain and white matter of the frontal lobe was found in the left hemisphere. The foci were in the mesodermal-glial organization stage. Remnants of necrotic tissue were visible in the center of the foci, and vessels, collagen fibers, and macrophages were visible at the edges. Some macrophages were filled with hemosiderin. At the periphery of the foci, a broad gemistiocytic shaft was visible. At the vessels, lymphocytic symptomatic reaction and loosening of the tissue structure were noticed. In the vicinity of the focus in the cortex, atrophy, ischemic disorders and fatty cell degeneration were evident. Vessels were maximally filled with blood. Fibrous and vitreous changes were observed in the walls of small vessels and atheromatous changes in larger vessels. The meninges were fibrotic. Single macrophages, lymphocytes, and histiocytic cells were present around the vessels.
