Clinical, anatomical and histological diagnosis
Histological diagnosis: Haemorrhagia secundaria od focum encephalomalatiae in regione nucleorum basalium sin. Autopsy examination of 78-year-patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin and Van Gieson's method.In the specimens from the basal nuclei, an extensive hemorrhagic focus was seen medially reaching the internal capsule in the region of the putamen and globus pallidus. In the center of the focus, single macrophages with hemosiderin deposits were found among red blood cells. The number of macrophages increases towards the periphery. At the border of hemorrhage with the surrounding tissue macrophages with glial cells and gemistocytes were observed. The surrounding tissue showed significant loosening of integrity, increased number of small vessels, and multiplication of glial nuclei. Macrophages without hemosiderin deposits, single or in clusters were seen. Sparse mononuclear infiltrates were seen around the vessels. Small vessels in the vicinity of the hemorrhage were filled with blood cells, arterial vessels had a thickened and fibrously altered middle membrane. Arteries with wider lumen showed separation and fragmentation of the elastic membrane. Perivascular spaces were markedly dilated. Numerous starchy bodies were visible. Neurocytes were absent around the foci. Thalamus nerve cells showed degenerative changes (steatosis). On the basis of the morphological picture, it can be assumed that hemorrhage occurred into the tissue previously altered by softening.
