Clinical, anatomical and histological diagnosis
Histological diagnosis: Arterio et arteriosclerosis gradus mediocris. Atrophia simplex. Status lacunaris. Autopsy examination of 78-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin, Heidenhain and Van Gieson's method.In analyzed specimens, significant thinning of neural cellular elements was observed in both cortex and brain stem, as well as strongly expressed features of ischemia in a significant proportion of cells. Generalized gliosis was visible. In the cerebral cortex, regional necrosis was seen. In the white matter of the cerebral hemispheres, sinuses of various sizes surrounded by starchy bodies were locally found; in the midbrain, partial disintegration of substancia nigra cells was seen; within the cerebellum, partial loss of Purkinje cells was observed. Medium-caliber vessels had thin smooth muscle layer and thick adventitia. Staining for myelin was less intense in areas of thinned tissue.
