Clinical, anatomical and histological diagnosis

Abstract:

Histological diagnosis: Encephalomalacia status lacunaris. Arterosclerosis cerebri. Autopsy examination of 77-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with with Hematoxylin-eosin and Van Gieson's method.The specimen taken from the focus showed necrotic tissue with reduced number of cellular elements, containing numerous circular hemorrhages. The cortex showed diffuse cellular desolation and locally incomplete necrosis. The cerebellum showed thinning of the granular layer. Severely advanced vascular atherosclerosis and obliteration of the layered structure of vascular walls, fragmentation and disintegration of the internal elastic membrane were observed. Small caliber vessels were often vitrified with overgrowth of the lumen. Several larger lacunae were visible in the basal ganglia at medium-sized vessels and extensive spongiosis with thinning of the tissue at capillaries.
