Clinical, anatomical and histological diagnosis
Histological diagnosis: Encephalomalacia incipiens incompleta in regione nucleorum basalium sinistrorum. Autopsy examination of 67-year-patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin and Van Gieson's method.At the level of the early left basal ganglia, the initial stage of incomplete necrosis was observed. Thinning of the striatum and pallium cells was observed, with various types of damage, and the staining of the tissue was reduced. In the vessels and capillaries, congestion was visible, and in the lower outer part of the pallium numerous extravasations were seen, filling the perivascular spaces. Near some vessels, clusters of leukocytes were found. Externally and downward from the pallium, more severe tissue disintegration was seen, probably around old, fibrotic cavity. The vessel walls were thickened, with features of fibrosis, and vitrified in some areas. The oligodendroglia of the white matter of the striatum was relatively well preserved, with features of glial cell activation within the inner capsule and proliferation and hyperplasia of the subependymal glia in the paraventricular region.
