Clinical, anatomical and histological diagnosis
Histological diagnosis: Haemorrhagia subarachnoidealis. Status lacunaris. Oedema, cerebri. Arterio et arteriosclerosis gradu majori. Autopsy examination of 72-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin, Heidenhain and Van Gieson's method.Extravasated blood was found in the left cerebral hemisphere in the subarachnoid space. The soft meninges of the brain were thickened, with prominent cellular reaction. A small hemorrhagic focus was present in the basal left part of pons. In the left basal nuclei, a necrotic focus without macrophage reaction was present, surrounded by a field of intense gliosis. In the central cortex, necrosis occupying the surface layers was observed. Features of edema and congestion and diffuse gliosis were seen throughout the brain. Abundant lipofusin deposits were found in the cells. Numerous medium-caliber arteries showed fibrous and vitreous lesions in hypertrophied myocytes. Numerous arterioles were vitreously altered.
