Clinical, anatomical and histological diagnosis
Histological diagnosis: foci encephalomalaciae multiplices partim haemorrhagicae. Status lacunaris. Atrophia cerebri senilis. Arterio et arteriolosclerosis gradu magno. Autopsy examination of 86-year-old patient was performed. Neuropathological evaluation in light microscopy w as based on brain paraffin sections stained with Hematoxylin-eosin, Heidenhain and Van Gieson's method.Different types of tissue disintegration were observed in the specimens: (1) fields of spongiosis with nerve cell dissolution (2) more advanced disintegration, showing a reticular stroma with numerous cell remnants, with sparse nuclei of astrocytes and even sparser nuclei of oligodendrocytes 3) the described above reticulum surrounded a hollow cavity 4) foci of hemorrhagic necrosis with macrophage infiltration and gemistocytic reaction of astroglia. The first three forms of necrosis were seen within the left central region, hemorrhagic necrosis in the left frontal region, and areas of spongiosis in the remaining cortex specimens. The preserved cortex was markedly thinned, sometimes with obliteration of layers. A focus of advanced decay without reaction was present within the head of the right caudate nucleus. In the cerebellum, there was significant thinning of the granular layer and Bergman's glial as well as partial loss of Purkinje cells. Extravasated blood was present in the ventricular lumen and subarachnoid space. Arterial vessels had thickened muscle layer, larger ones with atherosclerotic plaques occupying up to 50% of the lumen, smaller ones often fibrotic, quite frequent with vitreous changes...
