Clinical, anatomical and histological diagnosis
Histological diagnosis: Encephalomalacia cerebri inveterata. Status lacunaris. Autopsy examination of 78-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin and Van Gieson's method.In the left basal ganglia, an old malacia cavity was visible in the outer part of the putamen. Proliferation of connective tissue fibers was seen around this cavity. Loose hemosiderin granules were observed in the wall of the cavity. There were several smaller cavities in the rest of the ganglion, mainly in the inner capsule. Layer III and V of the island cortex were sparser in cells. The vessels of both the basal and pons ganglia showed extensive atherosclerotic changes. Symptomatic infiltrates of lymphocytes were present around the vessels. Perivascular lacunae were evident on both sections. An old scar with hemosiderin deposits was seen in the middle part of the pons base.
