Clinical, anatomical and histological diagnosis
Histological diagnosis: Malacia cerebri inveterata et cerebelli rubra recens. Arteriosclerosis cerebri. Autopsy examination of 65-year-old pa tient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin and Van Gieson's method.In the right frontal lobe there was a triangular focus of malacia destroying the white matter and spreading to the cortex, with partial preservation of the fine layer. In the central part of the focus a cavity was visible, containing remnants of disintegrating tissue, vessels and macrophages. The focus was in the phase of glial-mesodermal organization. The tissue structure, especially the white matter was very loosened. In the vicinity of the foci in the cortex, atrophy and cellular disorders of ischemic type were visible. Some cells showed features of lipofuscin accumulation. The vessels were hypertrophied, with swollen endothelium and single macrophages around the vessels were seen. There were small symptomatic vascular infiltrates of lymphocytic cells. Fresh hemorrhagic necrosis was observed in several areas of the cerebellum. Disintegrating tissue with obliterated structure with islets of macrophages was seen in the focus. In the vicinity, atrophy of the granular layer and Purkinje cells was evident. The vessels in both examined specimens presented large atherosclerotic lesions.
