Clinical, anatomical and histological diagnosis
Histological diagnosis: Lacunae postapoplecticae et cicatrices nucleorum basalium. Arteriosclerosis permagna. Atrophia corticis sinilis. Calcificationes intracerebralis.Autopsy examination of 70-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin, Heidenhain and van Gieson method.Large perivascular sinusoids and intra-tissue defects were found within the basal ganglia, mainly the putamen. These were filled with hemosiderin-laden macrophages, proliferating fibroblasts, and a loose network of collagen fibers. Within the globus pallidus, numerous medium-caliber vessels were visible with an intramural ring of pseudocalcium and pseudocalcium deposits lying loosely extravascularly. These same vessels had walls hypertrophied with collagen fibers, sometimes to the point of complete closure of the lumen. Very extensive cellular loss was found in the cortex of all areas; in the ammonal cortex, circular formations with small peripheral microglia seemed to correspond to senile plaques. The cerebellar cortex was relatively preserved. Large diffuse defects were present in the nucleus dentatus. Very advanced sclerotic changes were seen in vessels of all calibers. The glia were generally progressive, slightly thickened, but without hyperplastic forms.
