Histological diagnosis: Encephalomalacia recens in regione nucleorum basalium. Encephalomalacia inveterata cerebelli. Oedema cerebri. Ar teriosclerosis gradu mediocri. Embolia arteriae cerebri med.Autopsy examination of 47-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Cresyl-violet and Van Gieson's method.The left basal ganglia revealed an early stage of encephalomalacia. Extensive cell atrophy and prominent ischemic changes in the remaining cells were seen. Features of edema, loosening of tissue structure and widening of perivascular spaces were observed. Leukocyte infiltrates were present around the vessels, vascular endothelium was swollen, and hemorrhages were found locally. The glia was stimulated. In the cerebellum, a foci of malacia in the stage of advanced dissection and glial-mesodermal organization was observed, with numerous macrophages and a large connective tissue reaction. In the vicinity, the tissue structure was severely thinned, atrophy of the Purkinje cell layer and granular layer was evident. the Bergman glia was proliferating, the Purkinje cells showed features of chronic disease. The cerebral vessels were fibrously altered. There were blood masses separated by connective tissue elements in the lumen of the middle cerebral artery.
