Clinical, anatomical and histological diagnosis

Abstract:

Histological diagnosis: Glioblastome multiforme.Autopsy examination of 70-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin and van Gieson method.The tumor was found exhibiting a looser cellular composition in the peripheral parts and was characterized by tremendous polymorphism. Multinucleated cells, tumor hypertrophied astrocytes with a rim of nuclei at the periphery, nuclear and hyperchromatic conglomerates, large spindle cells, in addition to small round poorly differentiated and numerous small spindle cells were observed. In the center, the tumor presented clumped cell arrangements, with a predominance of small hyperchromatic and poorly differentiated cells. However, between them there were numerous cells with large dark nuclei and very numerous /some in the field of view/ abnormal mitoses. In one place the tumor infiltrated the cortex extremely densely, passed to the meninges and through the meninges to the neighboring gyrus. The cellular packing was very dense and the low differentiation of lymphocyte-like and spindle cells in this area required differentiation with sarcoma. The ratio to vessels is against the latter: vascular wall overgrowth was found almost nowhere. A free space remained between the vessel and the tumor tissue.
