Clinical, anatomical and histological diagnosis
Histological diagnosis: Encephalomalacie multiplices. Atrophia cerebri gradu mediocri. Arteriosclerosis cerebri.Autopsy examination of 63-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections (standard stainings) and brain frozen sections stained with Bielschowsky method.Massive cellular changes were found mainly affecting the frontal, central cortex consisting of ischemic, chronic disease and fatty degeneration. The vacuolations occurred mainly perivascularly. Vitreous and fibrous lesions were seen in the vessels. Lymphocyte-like cells, macrophages with hemosiderin, and red blood cells were seen adjacent to the vessels. Single lacunae were seen in the basal ganglia, mainly in the striatum. In the hippocampus, a small early focus of malacia was found. Besides, features of congestion, activation and proliferation of glia were observed throughout the nervous system.
