Clinical, anatomical and histological diagnosis
Histological diagnosis: Encephalopathia uraemica simplex.Autopsy examination of 20-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin, van Gieson and Heidenhain method.The most advanced changes affected the cortex of all sampled areas. Focal perivascular and diffuse desolations leading to obliteration of layered pattern were found. The atrophied neurons were replaced by necrosis. Cortical capillaries were mediocrely filled, perivascular spaces were well defined. Venous stasis was observed in the white matter. Myelin preparations showed myelin sheath edema. In the basal ganglia, perivascular spaces were quite significantly widened; vessel walls were hypertrophied. Several small circular hemorrhages were found in the pons. Intravascular lympho- and leukocytic thrombi were seen in some vessels. The meninges were edematous.
