Clinical, anatomical and histological diagnosis
Histological diagnosis: Atrophia senilis corticis cerebelli. Status lacunaris et cribrosus nucleorum basalium. Arteriosclerosis.Autopsy examination of 72-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin, Heidenhain and van Gieson method.The cortical layer was narrow. Loss of neurons was evident. In the basal ganglia /putamen, thalamus/, vascular lacunae and very large empty subcortical lacunae were found. Sponge-like thinning was observed at small vessels. Extensive vascular vacuolations were also seen at all levels in the white matter. The arteriosclerotic lesions were systemic. In the cerebellar cortex, thinning of Purkinje cells and significant, but not complete, atrophy of the granular layer were seen. The glia generally showed a weak response, and tended to be thinned and regressive in the basal ganglia. There was some densification of elements in the white matter of the frontal and parietal regions, but progressive forms were found rather sporadically. Prominent steatosis of basal ganglia cells was found in the bulb, in the cranial nerve nuclei. In addition to the accumulation of lipopigment, vacuolar changes were also present.
