Clinical, anatomical and histological diagnosis

Abstrakt:

Autopsy examination of 36-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin, van Gieson, and Heidenhain method. On the thoracic spinal cord section, which macroscopically appeared to be an extradural tumor overgrowing the proper spinal tissue, an image of a bordered deformity was found on microscopic examination. Namely, a "butterfly" of gray matter was stretched between two parts of the anterior and posterior white matter. The anterior groove and posterior septum of the medulla were absent. One anterior horn was in conjunction with this separating gray zone, the other in the form of an ectopic group of cells lay at some distance from the central focus. The spinal canal was located laterally. The inferior medullary segment showed atrophy within the gray and white matter. In addition, a picture of radiculitis and intervertebral ganglia was evident. The myelin sheaths were swollen and partially or completely disintegrated. The changes within the roots and ganglia were quantitatively different at different levels. Myelin swelling and disintegration also occurred in the rostral zone of the medulla. Within the brainstem and hemispheres, mediocre edema, vascular stasis, and single transudative elements in the vascular spaces were observed.Histological diagnosis: Polyganglioradiculitis in inviduo cum mafornatione congenitale medullae spinalis thoaralis.
