Clinical, anatomical and histological diagnosis
Autopsy examination of 9-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin, van Gieson and Heidenhein method. The atrophic lesions in the cerebral cortex were relatively mild, diffuse, patchy, and maintaining a stratified pattern. Very severe changes in the cerebellum in the form of acute edema were observed. The granular layer of the cerebellum was very severely thinned. The " hepatic" glia was very poorly represented in the white matter of the hemispheres in the paraventricular region, but there were quite a lot of normal, slightly activated astrocytic nuclei. In the brainstem in the aquaeductus cerebri region and in the basal nuclei there was an enormous accumulation of naked Alzheimer nuclei. Numerous cells with lipopigment deposits and degenerated cells were present in the thalamus. Edema was found among the fibers of the capsula interna - a "hepatic" glium was very abundant near the affected cells of the thalamus.Histological diagnosis: Atrophia granularis incipiens cerebelli. Oedema cerebelli. Atrophia disseminata corticis cerebri.
