Abstrakt:

Histological diagnosis: Atrophia cerebri senilis cum angiopathia senile. Arteriosclerosis cerebri cum status lacunaris. Focus encephalomalaciae recens in regione pedunculi cerebri. Autopsy examination of 82-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin and van Gieson.In the pons, the edge of the foci of softening with dissolving tissue was visible. Scattered and in small clusters leukocytes and macrophages were present in the focus. At the periphery of the focus, locally dense cellular gliosis with the presence of progressive forms was observed, without connective tissue reaction.The vascular changes observed in the specimens were in the form of:1. atheromatous lesions 2. vitreous and vitreous-fibrous changes in medium-sized vessels;3. proliferation of cellular elements of vascular walls, mainly in smaller vessels;4. the presence of vascular convolutes seen in both the frontal region section and the basal ganglia.In addition, sinusoids and perivascular thinning of the tissue structure and quite numerous symptomatic lymphocytic infiltrates were found. In the cortex, diffuse and widespread atrophy of nerve cells with obliteration of the stratified tissue, fatty degeneration of nerve cells /also in the ganglia and brainstem/ and parenchymal damage similar to ischemic or chronic disease were found. Glial thickening in the white matter gave the impression of gliosis from shrinkage rather than reactive gliosis, with, however, the presence of few progressively altered forms of astroglia. In addition, features of stasis and edema were observed.
