Clinical, anatomical and histological diagnosis
Histological diagnosis: Kernicterus.Autopsy examination of 4-month-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with crezyl-violet, Kanzler-Arendt and Heidenhain method.There was an almost complete lack of cells in the pallidum; the few remaining cells were sclerotically altered. Myelin staining showed a paleness of this structure due to insufficient myelination of the short tracts of the pallidum. The mirror image of cellular depletion and myelin paleness was intense fibrous gliosis. Cellular thinning was also evident in the hypothalamic nucleus of Luys, which was surrounded laterally by a glial scar, partially involving the nucleus itself. Lesions were also present in the cells of the dentate nucleus in the cerebellum. Within the pallium, cellular loss and lesions were most prominent in the amonal region and entorinal cortex, and neuronal damage was more severe in the neocortex. In the brainstem, motor cells /N VII/ were well preserved; there was diffuse damage in the cochlear nucleus and associative-vegetative cells, especially under the ventricle IV; fibrous gliosis was also marked in this area.
