Clinical, anatomical and histological diagnosis
Histological diagnosis: Poly - radiculomyelo – encephalopathia.Autopsy examination of 32-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin and van Gieson, Heidenhain, Kanzler-Arendt and cresyl violet method.The whole microscopic image of the specimen showed no signs of inflammation. Swelling of the nerve sheaths and fibers, vascular congestion, and obliteration of the wall structures were seen in the retrieved nerves and radicles. At the low cervical level there was spongiform necrosis progressing to diffuse necrosis involving the medulla hemidiaphragmatically. Atrophy of anterior horn neurons and cellular disease of residual neurons was evident at all lower and higher levels of the cord. In the pons, atrophy was seen in nuclei XII, X, VIII vestibularis and in the reticular substance. Thinning of the cerebellar granular layer, segmental atrophy of Purkinje cells and nucleus dentatus neurons were observed. There were generalized cellular changes in the brain - ischemic type diseases /mainly cortex/, severe and acute edema /thalamus/. Fields of limited spongiform necrosis without reaction were present, scattered at different levels. Perhaps they should be linked to respiratory distress and stasis in the final preagonal period of the disease.
