Clinical, anatomical and histological diagnosis
Histological diagnosis: Foci encephalomalaciae, multipl. partim cum haemorrhagiae. Status lacunaris. Arteriosclerosis cerebri.Autopsy examination of 86-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin and van Gieson method.The basal ganglia showed a symmetrical focus of malacia in demolition-phase with secondary hemorrhage in the left ganglia. Lacunes were present in the putamen and globus pallidus. In the cerebellum, foci of malacia occupying several convolutions were present, destroying mainly the granular layer and the white matter. The fine layer was generally unaffected. Purkinje cell atrophy, an ischemic type disorder, was found in adjacent convolutions. In the midbrain, a demolition focus was found destroying the area of the oculomotor nerve. Glial activation and proliferation were evident. Features of edema and congestion were observed. Infiltrates were observed in the meninges, especially in the vicinity of necrotic foci, consisting mainly of macrophages and lymphocyte-like cells. The vessels showed vitreous-fibrous changes.
