Clinical, anatomical and histological diagnosis
Histological diagnosis: Encephalomalacia cerebri. Arteriosclerosis cerebri.Autopsy examination of 77-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin and van Gieson method.A focus of diffuse necrosis was seen in the frontal lobe, destroying the white matter and cortex over a large area, preserving the fine layer almost completely. The focus was covered with macrophages, with little vascular reaction, causing complete tissue disintegration in places, and a spongy state in places. The cortex showed extensive neuronal atrophy and obliterated layered pattern. The activation and proliferation of the glia was evident, especially in the vicinity of the focus. Small macrophage infiltrates were evident in the meninges; vessels were hypertrophic with fibrous changes.
