Clinical, anatomical and histological diagnosis
Histological diagnosis: Foci Encephalomalacia in regione hemisphaerii cerebelli sinistri atque hemisphaerii dextri cerebri. Thrombosis arteriae cerebri mediae dextrae et encephalomalacia recens suspecta.Autopsy examination of 70-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin and van Gieson method.Several different timed foci of malacia were found in the examined material. In the left cerebellar hemisphere, a well-developed dissection focus with reparative process involving all layers was seen. Within the malacic focus, there were occasional clusters of macrophages with hemosiderin. A layer of Bergmann's glial cells was well preserved. In the parietal region of the right cerebral hemisphere, further foci of malacia in the stage of well-advanced dissection and repair with numerous fibrous elements and macrophage clusters were seen in the white matter and cortex. Leukocytic infiltrates were present locally around the periphery of the foci. The protoplasmic glia was stimulated. Extensive cellular vacuolation was found in the cortex, cells were pale and damaged. Masses of clotted blood morphotic elements: red blood cells, lymphocytes, and leukocytes were seen in the right middle cerebral artery. The arterial wall was atheromatically altered with a thickened elastic membrane, an inner membrane that was stratified over much of the circumference, and a thinning of the muscular layer.
