Clinical, anatomical and histological diagnosis
Histological diagnosis: Encephalomalcia cerebri alba et rubra.Autopsy examination of 64-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin. In the left basal ganglia, a large foci of malacia was found, involving the head of the caudate nucleus, the putamen, going as far as the insula. The focus, in the stage of dissection or glial-mesodermal organization, was prominently hemorrhagic in places, thus numerous circular hemorrhages, hemorrhages by diapedesis were present. Non-hemorrhagic necrotic tissue contained numerous macrophages, a rich network of young vessels, as well as developed, hypertrophic vessels. The glia were significantly activated, with gemistocytes present in some places. Symptomatic infiltrates were prominent. Neuronal atrophy, ischemic or chronic cellular disorders were observed in the insular cortex. The vessels showed fibrotic changes. Besides, edema and congestion were found throughout the nervous system.
