Abstract:

Autopsy examination of 59-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin and van Gieson method. In the cerebral hemispheres, intense features of edema were noted - particularly prominent in the cortex in the white matter and in the basal nuclei. In the brainstem area the intensity of edema was lower, and a marked widening of perivascular and pericellular spaces was observed. Nerve cells in the cortex and basal nucleus were shrunken, dark, in broad cavities, with a probable putrefactive component. There was a localized focus of fresh necrosis in the frontal cortex, surrounded by very thinned tissue. The amount of glia was not increased. The glial cells were shrunken, dark. Only in the dentate nucleus of the cerebellum single, large, bright glial cells of type - Alzheimer II were seen. Diffuse necrosis of the granular layer cells was present in the cerebellum. Purkinje cells were preserved, mostly shrunken, dark, with pyknotic nuclei. Despite the putrefactive component, the perceived lesions appear to be consistent with a picture of prominent edema and its consequences. The hepatic encephalopathy component was negligible.
