Clinical, anatomical and histological diagnosis

Abstract:

Histological diagnosis: Encephalomalcia cerebri inveterate. Status lacunaris. Arteriosclerosis cerebri.
Autopsy examination of 77-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin and van Gieson method. In the left basal ganglia, an old focus of malacia at various stages of glial-mesodermal organization was found. The focus occupied the putamen, the external capsule, the last insula, the upper part of the internal capsule, and part of the head of the caudate nucleus. Large and small perivascular cavities were present in the putamen, occasionally filled with macrophages and loose devascularized network. The vessels showed vitreous-fibrous changes. Infiltrates of lymphocyte-like cells and macrophages were observed in the meninges adjacent to the focus. Whole brain revealed features of edema and congestion.
