Description:

Clinical, anatomical and histological diagnosis

Abstract:

Histological diagnosis: Meningo-encephalitis purulenta /necrotoxica?/. Encephalomalacia alba partim rubra. Arteriosclerosis cerebri. Autopsy examination of 82-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin and van Gieson method. The meninges showed prominent leukocytic infiltrates, with admixture of histo-lymphocyte-like and acidophilic cells. A frontal lobe specimen taken from the edge of the foci of malacia revealed, mainly in the cortex, localized leukocytic clusters. The leukocytes were located throughout the neural tissue or perivascularly, but mainly in the necrotic areas or in their margins. Besides, hemorrhages by diapedesis and circular hemorrhages, neuronal atrophy locally obliterating the layered structure, and cellular disorders of ischemic and chronic type were seen in the cortex. The white matter structure was relaxed, disintegrated in places, and had the character of a spongy state in places. The vessels were hypertrophied and fibrously altered.
