Abstract:Histological diagnosis: Lacuna inveterata medullae oblongatae. Necrosis incipiens nucleorum basalium. Arteriosclerosis. Autopsy examination of 76-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin and van Gieson staining.The basal ganglia showed loss of neuronal elements, myelin dissolution, regressive oligodendroglial changes, and progressive astrocytes and endothelium. Astrocytes were large and numerous. The surrounding vessels were altered atheroscleromatically, with wide lacunae. In the bulb, a normally shaped olive was visible on one side, and on the other side a lacuna with edges reinforced by proliferating glia and smooth walls was visible; in the center, remnants of the vessel and a loose collagenous tissue were found. The position of the lacuna corresponded to the Wallenberg focus, but was slightly too vertical; however, this could be due to secondary distortion of the material.
