Autopsy examination of 72-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin, PAS and van Gieson method.A specimen from the right basal ganglia revealed a focus of fresh necrosis without tissue disintegration, involving the insular cortex, external and extrema capsule, claustrum, striatum white matter semioval center and partly internal capsule, without clear demarcation borders. Throughout the area a large loss of cellular elements, both neurocytes in the striatum, cortex, and glial cells were seen. The remaining cells were all severely damaged, regressive forms of gliosis, and ischemic changes were observed. The white matter showed atrophy of glial cells and stimulated vascular endothelium. In the gray matter, scattered single macrophages were seen in some areas. In the thalamus, atrophy of neurocytes and extensive damage to preserved cells due to accumulation of fatty substances were also observed. The vessel walls showed fibrous changes.
